I/We the undersigned hereby certify that I/We am/are the parent(s) or legal guardian(s) of the trip.
Consent for medical treatment:
I/We hereby give permission for the staff of the Trip Mic’24 PSI to seek appropriate medical attention for the members of the trip and for the medical attention to be given and for the participants to receive medical attention in the event of accident, injury or illness. I/We will be responsible for any and all costs of medical attention and treatment, except for that covered by the tournament’s excess medical coverage policy.

Athletics Release:
I/We hereby acknowledge that everybody in my family that is taking part of this trip is physically fit and mentally capable of participating in the trip activities. I/we hereby acknowledge that playing or practicing soccer and doing other activities planned in the trip are potentially hazardous activities. I/we agree that I/we am/are voluntarily entering and assuming all risks associated with participating in those activities, including but not limited to falls, contacts with other participants, the effects of weather including heat and or humidity, and the conditions of the training grounds and facilities, all risks being understood and appreciated by you. I/We, the undersigned for ourselves, our heirs, executors and administrators hereby waive, release, and forever discharge Pro Skills International Soccer Academy LLC and their stuff, officers, agents, employees, representatives and successors and assign of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in this trip or tournament, whether or not damages, injury or loss is due to negligence. I also hereby acknowledge that no refunds will be given due to any type of injury occurring during the trip.
Use material for promotional purposes
I/We hereby give permission to Pro Skills International Soccer Academy LLC to use pictures and videos of the travelers/ players/ families/ participants to promote future events organized by the company.

Payment Terms
I / we know that if we choose making the payment in 3 payments we compromise to make the other 2 payments in time. Otherwise I/ we will miss the right to participate and the money paid.

Weather Policy:
In the event of inclement weather once trip begins, all participants will be flexible to posible changes, re schedules or cancellations.

Itinerary:
The itinerary is flexible and can be modified. Activities planned can be eliminated, modified, rescheduled, canceled or adapted.

Clothing:
The players will be dressed as considered by the coaches / staff for the events/ training/practices/ games during the tournament. Participants will be flexible with any mistakes in sizes in uniforms. We need to understand that sizes in Europe and the USA are different.

Allergies:
PSI will help the participants to avoid any kind of allergy and eat safely but we I/We know that ProSkills International Soccer Academy LLC is not responsible for any allergy or medical problem during this trip.

Refund Policy:
The fees are non-refundable, however the total amount you originally paid for is guaranteed and the value of the registration will be credited towards a later trip or it can be transferred to another traveler (valid until 01/01/2024).
